Schistosomiasis is a parasitic disease caused by flatworms that live in the blood vessels surrounding the bladder or intestine (depending on the species). Infection occurs when larvae in fresh water penetrate the unbroken skin of people coming into contact with that water. Infected people contribute to transmission of the parasite when eggs in their urine or faeces enter fresh water: the eggs hatch, releasing miracidia (another larval stage of the parasite). Miracidia seek out, and infect, snails in the water. An infected snail subsequently releases hundreds of cercariae per day - each one capable of infecting a person and developing into an adult worm.

One of my PhD projects was a systematic review and meta-analysis to investigate whether schistosome infection status was associated with access to safe water and adequate sanitation, and the use of soap. we used reandom-effects meta-analysis to synthesise different studies’ findings. We assessed publication bias (the risk of studies’ findings affecting their likelihood of being published) through the visual inspection of funnel plots and through Egger’s test. Heterogeneity of datasets (the overall spread of different effect sizes in different studies) was quantified using Higgins’ I2 and investigated in subgroup analysis.
Safe water supplies were associated with significantly less schistosomiasis (odds ratio (OR) = 0.53, 95% confidence interval (CI): 0.47–0.61), adequate sanitation was associated with less infection with Schistosoma mansoni (the parasite causing intestinal schistosomiasis in Africa and Latin Ameria), (OR = 0.59, 95% CI: 0.47–0.73) and Schistosoma haematobium (the parasite causing urogential schistosomaisis in Africa and the Middle East) (OR = 0.69, 95% CI: 0.57–0.84). No studies were found providing eligible data on hygiene practices and schistosome infection.
